Reporting Overview

Injury/Illness
Report all injuries, illnesses, and exposures suffered by USF faculty, staff, students, visitors, and volunteers while in the course of their employment, volunteerism, course of study, visiting USF locations, or participating in a USF sponsored event
Spills/Releases
Immediately report major spills or releases of hazardous materials to EH&S at 813-974-4036. Outside of normal business hours, call University Police at 813-974-2628.
Fire
Report all fires. Contact Risk Management to discuss the process for an insurance claim, if applicable. If there are injuries involved, also report these.
Property Damage
Report property damage. Contact Risk Management to discuss the process for an insurance claim, if applicable.
Vehicle/Cart Accident
Report all vehicular accidents while driving a state-owned vehicle, or while driving a rental vehicle or personally-owned vehicle while on University business. Contact Risk Management to discuss the process for an insurance claim, if applicable. If there are injuries involved, also report these.
Other Reportable Event
Contact Risk Management about other reportable events that involve summer camps, law enforcement, automobile insurance, boiler & machinery, contractors' equipment, crime coverage, electronic data processing equipment, fine art, or ocean marine.

Unsafe Conditions
Wet or slippery floors, cracked or uneven sidewalks, obstructed exits or fire lanes, mold, pests, fallen tree branches
Unsafe Acts
Not wearing PPE, creating trip hazards, not having fall protection, unsafe operation of vehicles or equipment
Close Calls

What to Report
Financial fraud/waste, retaliation, human resources issues, discrimination, sexual harassment, research misconduct
Work-related Employee/Volunteer Injury & Illness Reporting
Faculty, staff, and volunteers who are injured, exposed, or become ill while at a USF location or participating in a USF sponsored event must report all incidents to their supervisor, no matter how minor.
